107.12 Contractor's Responsibility for Utility Property and Services
This article is to reinforce the importance of consistent practices when validating the presence of a licensed electrician on projects during the performance of electrical work.
State law requires the presence of a licensed electrician on the job site when work is being performed on state projects involving electrical wiring.
Project offices shall require the contractor to furnish the electrician’s name and a copy of his/her license at the pre-construction meeting. We encourage staff to request certification from the contractor, at the pre-construction meeting, that a licensed electrician will be on site when performing electrical work.
The Department’s freeway lighting system, the ITS, and other miscellaneous electrical systems are not a part of Miss Dig. The Contractor is to contact the maintenance representative at the MDOT Region Office before starting work near lighting systems, the ITS, and traffic systems. Work is not permitted to start until the Department has staked the lighting systems, the ITS, and traffic systems. A single statewide staking form (Form 5300) is live on the MDOT website (https://mdotcf.state.mi.us/public/webforms/public/5300.pdf).Form 5300 This form covers staking requests for the following MDOT underground infrastructure: ITS, Traffic Signals, Roadway Lighting, and other miscellaneous electrical. The requester submits the form by Region from selecting the Region hyperlink where the work is being performed. The hyperlink pre-populates an email with all Region utility contacts.
These emails should be monitored and requests should be disseminated to the appropriate contacts (direct forces / third-party contract) to complete the staking request. The current requirement is for the requester to submit the form at least 5 work days prior to the anticipated start date of excavation work (excludes weekends and holidays). Excavation work is prohibited until the requester is notified that the staking request has been completed.
The frequently used special provision (FUSP) 12SP-800B-01 (Staking Michigan Department of Transportation Underground Electrical Infrastructure and ITS Facilities) directs the Contractor to complete Form 5300 and to also contact the regional staking representative(s) 72 hours prior to commencing work.
