A constructability review by both development and delivery staff is essential in ensuring that those items with the greatest potential for impacting project cost or delays are addressed during plan preparation.  To facilitate these reviews and provide accountability, constructability checklists have been developed.  Review procedures have been incorporated into the Scoping Manual and the Road Design Manual.  In addition, Program/Project Management System (P/PMS) network tasks have been developed to track preliminary and final constructability reviews for all road and bridge projects.
 
 
 
 
 
 
 
Preliminary Constructability Review
 
 
 
The Checklist for Constructability Review – Early Project Scoping (Form 1961) must be completed, signed by both development and delivery staff, and included as a part of the Scoping Package turned in during the Call for Projects process (which is prior to beginning preliminary plan development).  Further guidance on Preliminary Constructability Reviews can be found in Chapter 7 and Appendix B-10 of the Scoping Manual and Section 14.30 of the Road Design Manual.
 
 
 
 
 
 
 
Final Constructability Review
 
 
 
The Checklist for Constructability Review – Project Development Phase (Form 1960) must be completed and signed by both development and delivery staff prior to plan completion. The Final Constructability Review will be performed after the Plan Review, in conjunction with the development of final plans and specifications. All pay estimates require an original signature by the delivery engineer no later than 48 hours after generation. This signature may be delegated to the assistant delivery engineer if they have been assigned to the project. However, anytime an assistant delivery engineer signs an estimate, their signature must include “signed for” under the typed name of the official delivery engineer in responsible charge of the project. A signed original must be kept in the project files and/or digitally signed pay estimates in the project’s electronic construction directory within ProjectWise.
 
 
 
 
 
 
 
Prevailing Wage Quality Assurance Review Procedures
 
 
 
There are three levels of quality assurance (QA) reviews within the construction administration process to ensure proper procedures and documentation is used in the oversight of the prevailing wage program. The levels include:
 
 
 
    • Local agency project oversight
 
 
 
    • Transportation Service Center (TSC) project oversight
 
 
 
    • Statewide program oversight
 
 
 
 
 
 
 
The following procedures outline the process for QA reviews for prevailing wage oversight. Conduct QA reviews each construction season at each level to verify appropriate oversight is being accomplished and documented on federal and state prevailing wage projects. It is recommended the QA review take place during the active portion of the construction project. This allows for implementation of corrective action, if necessary, for the remainder of the project and subsequent projects. Document and discuss all deficiencies and issues found during the QA review with the delivery engineer. Do follow-up reviews to assess if the deficiencies and issues have been addressed. MDOT Form 1965, Prevailing Wage Quality Assurance Review Checklist, is available to assist with the QA review, and is available on MDOT’s Web site. You can use the form for each level of review. Upon completion of the QA review, prepare a cover letter summarizing the findings and attach the completed checklist(s) to it; distribute as indicated on the checklist form.
 
 
 
 
 
 
 
Local Agency Level
 
 
 
The TSC delivery engineer is responsible for performing the QA review at the local agency project level. In some regions, the region coordinator for prevailing wage, local agency engineer or local agency technician may assist the delivery engineer. Complete a minimum of one QA review for each local agency that has a construction project during the year. Additional reviews may be necessary if problems are found with the program oversight and documentation by the agency or their consultant. If TSC personnel complete the QA review, forward a copy of the review findings to the region coordinator for information, and for coordination with the TSC of possible follow-up actions. The region coordinator is to forward a copy of the findings to the Construction and Technology (C&T) Division’s prevailing wage compliance specialist.
 
 
 
 
 
 
 
TSC Level
 
 
 
The region coordinator for prevailing wage is responsible for conducting QA reviews for each delivery engineer in each TSC. Review a minimum of 10 percent of each delivery engineer’s projects, with a minimum of one project if the engineer has ten projects or less. The region coordinator is to forward a copy of the findings to the C&T Division’s prevailing wage compliance specialist.
 
 
 
 
 
 
 
Statewide Level
 
 
 
The C&T Division’s prevailing wage compliance specialist is responsible for conducting QA reviews to assess the prevailing wage compliance program for consistency and uniformity across all areas of the state, and to determine training needs for the TSCs and local agencies. This assessment will be completed using the reports provided by the various region coordinators for prevailing wage, and by conducting independent QA reviews across the state. A minimum of one review will be completed in each region. In addition, one consultant administered project will be selected for QA review. Additional reviews may be added based on the findings from the statewide review and/or the information contained in the review findings provided by the region coordinators for prevailing wage.
 
 
 
 
 
 
 
Wage Rate Interviews
 
 
 
Construction engineers are required to conduct annual wage rate interviews on all prevailing wage federal and state funded projects during the construction season.  Contractors are to instruct their employees to cooperate during the interviews and provide the requested information.  Per the prevailing wage oversight procedures, wage rate interviews (WRI) are required to be performed at least once for each contractor and subcontractor when they are on the job, throughout the life of the project, for both state and federally funded projects.  Conduct the interviews each season with a representative sample of the work force for each contractor and subcontractor on the project.  Include each classification and group in the interview process.  Operators of equipment and trucks whose ownership is not clearly identified should be interviewed.  Use Form 1156-T1 (Field Review) for this purpose. These interviews are to assure that all operators, laborers and other skilled trades are paid the base hourly rate and fringe benefits as specified in the project proposal.
 
 
 
 
 
 
 
The engineer will compare the wages and the work classifications documented on Form 1156-T1 with the certified payrolls, IDRs, inspector’s observations, diaries, documentation in the project files, and prevailing wage compliance requirements in the contract.  Form 1156-T1 must be used and properly completed.
 
 
 
 
 
 
 
The construction engineer must resolve any deficiencies or discrepancies (i.e., an employee is not paid the prevailing wages and fringe benefits, as stated in the proposal) utilizing the prevailing wage oversight procedures in BOH IM 2009-07.  If the problem cannot be resolved or additional information is needed, contact the Prevailing Wage Compliance Specialist in the Division of Construction Field Services.  If additional worker information is needed, fill out Form 1156-T2 for documentation.  This form is available on MDOT’s Web site at:

Oversight of Maintenance Funded Projects (M Jobs)
 
 
 
These projects require the use of FieldManager for record keeping and documentation.  Projects in this category are let under the MDOT Standard Specifications for Construction and the referenced manuals and guides. Oversight and documentation requirements for these projects are the same as any state or federally funded construction project. The same procedures for inspection, pay item documentation, quality control/quality assurance testing, and materials testing must be followed.  The procedures for final inspection/acceptance and submission of the final estimate remain the same as for standard construction projects. In order to process the final estimate, the project records will need to have a final estimate review in accordance with Bureau of Highways Instructional Memorandum 2011-4, Final Estimate Review Procedure, unless the overseeing engineer is certified under the current MDOT Engineer Certification Program.

Documenting Price Negotiations
 
 
 
Federal regulation states, “The SHA (State Highway Agency) shall perform and adequately document a cost analysis of each negotiated contract change or negotiated extra work order”.  In addition, cost analysis documentation is required for all adjustments to the contract. When using negotiated unit prices the analysis, justification and documentation should include all applicable information from the following:

2. Negotiated unit prices based upon force account type records
 
 
 
When the negotiated unit price method is not available or the unit price cannot be determined prior to completion of the work, and it is shown that the cost is less than the amount if computed using actual force account records, unit prices can be supported using force account type records.  Documentation for this method is to include labor, equipment and material usage, and cost totals.  Convert to a unit price before adding on administrative costs, profit and other costs.  When using this method to document the cost of extra work, include documentation with the contract modification for extras totaling $25,000.00.  For extra work less than $25,000.00, keep the documentation in the project files and cross reference the file location on the contract modification. 
 
 
 
 
 
 
 
3. Force account procedures
 
 
 
When unit prices cannot be agreed upon in advance and the force account type record method is not available, use force account procedures to determine the cost of the extra work.  Subsection 109.07 of the 2003 Standard Specifications for Construction contains force account procedures. Subsection ??109.05.D.?? of the 2012 Standard Specifications for Construction contain force account procedures. 
 
 
 
 
 
 
 
Documenting Impacts to Contract Time
 
 
 
Staff need to address the impacts on contract time from extra work, quantity changes, and adjustments to the contract.  The contractor completes an analysis of the time impact for extra work and submits it to the engineer as part of the Work Plan under ??109.07.A?? of the 2012 Standard Specifications for Construction. Include extensions of time in the contract modification, when warranted, and if it can be determined at that point.  If the duration of the extension of time cannot be determined when the contract modification is prepared, document whether an extension of time is warranted on the contract modification.  Any extension of time will be determined upon completion of the extra, additional, or adjusted work.  In all cases, use Section 108 of the 2012 Standard Specifications for Construction to determine the extension of time to be granted.

Independent Assurance Testing (IAT)
 
 
 
IAT requirements are outlined in Section 5 of the Materials Quality Assurance Procedures Manual.  The independent assurance testing program is an integral part of the construction materials sampling and testing program that has been mandated for state Department of Transportations by the Federal Highway Administration (FHWA). Per 23 CFR 637, independent assurance program is defined as “activities that are an unbiased and independent evaluation of all the sampling and testing procedures used in the acceptance program”.
 
 
 
 
 
 
 
The following items have been identified as needing special attention:
 
 
 
# It is essential that ALL qualified (refer to Section E.1.3 of Reference for explanation of the term “qualified”) sampling and testing personnel should be a part of the independent assurance (IA) tests. Likewise, equipment involved in acceptance sampling and testing should be used for IA tests. The department is required routinely to verify to both federal and state authorities that these requirements are being met as part of our obligations for federal transportation funding.
 
# All Region IAT Coordinators should ensure that the IAT database is continually updated. Copies of IAT reports are distributed to appropriate areas.
 
# Refer to Section E.5 “IAT Technician” of Reference, which states that “The IAT coordinator will assign qualified personnel to conduct IATs. IATs should be performed by technicians not normally involved in the acceptance testing for the project. At the discretion of the IAT coordinator exceptions may be made due to circumstances such as remoteness of the project or staffing limitations. This should be the exception rather than the rule.”
 
# Region materials personnel should remain vigilant regarding potential conflict of interest issues with consultant personnel on IATs. A consultant firm cannot be participating in IAT oversight of tester/s working for the same firm.
 
 
 
 
 
 
 
 
 
 
 
 
 
 
 
Construction and Technology Indirect Testing Credit
 
 
 
Materials testing and investigation is an essential component to the overall operations of MDOT. There are a number of reasons for materials testing, including verification and qualification of material products of potential vendors, inspections of labs and facilities in promulgation of testing and acceptance of products, and field testing to ensure compliance with standard material specifications. The indirect testing template program detail reads:
 
 
 
 
 
 
 
<nowiki>The [C&T Indirect Testing Credit] funds are used for MDOT personnel or authorized agents for testing performed in materials testing labs throughout the state that are not project specific, such as certification and verification of materials used in MDOT projects, stockpile testing, testing materials of pre-qualified material producers, soil testing, salt testing, and host of other non-project specific testing that is required to maintain the integrity of the materials used in the Trunkline maintenance and construction.</nowiki>
 
 
 
 
 
 
 
The purpose of the Construction & Technology indirect testing credit is to cover performance, material, and acquisition expenditures that relate to either multiple projects, capital program administrative testing, or to perform testing related work for an un-programmed project wherein the activity is not scoping in nature (please refer to the MDOT Project Scoping Manual.
 
 
 
 
 
 
 
Eligibility and Use
 
 
 
Following are examples of types of expenditures that would be appropriate to code to the Construction and Technology indirect testing credit project number. It is noteworthy to state this is not an all-inclusive list, as operational needs are consistently in a state of change and require adaptation.
 
 
 
* Prequalification of interested vendors (salary and travel).
 
* Certification/verification of samples taken at a vendor’s lab/facility (salary and travel).
 
* Certification/verification of samples taken in the field if a project number is not available (salary and travel).
 
* Construction season closing process, including preparation of forms, documentation, manual updates, etc. (salary and travel).
 
* Lab preparation for a future construction season (salary).
 
* Lab supplies and materials required for testing (other operating expense).
 
* Lab equipment required for testing (other operating expense).
 
* Lab supplies and materials required for research (other operating expense).
 
* Calibration related expenses for testing and research equipment (salary, travel, and other operating expenses).
 
* Vehicle and equipment usage (other operating expenses).
 
* Lab qualification activities, such as proficiency testing.
 
* Lab accreditation activities.

Inappropriate Use
 
 
 
It is not appropriate to use the Construction and Technology indirect testing credit to substitute or supplement administrative costs, as a substitute for small increments of activity that occurred on multiple programmed projects, or avoidance of multiple lines of coding.

Buy America Provision
 
 
 
The Buy America regulations require the use of domestic steel and iron in all federally funded highway construction (including local agency and transportation enhancement projects) where the material is permanently incorporated into the project.  Buy America covers all foreign steel and iron materials and products, regardless of the percentage they comprise in a manufactured product or the form they may take.  This also applies to steel and metal material used as a minor component with other manufactured products, e.g. reinforced concrete pipe.

Waivers
 
 
 
The contractor may request a waiver on certain materials for contract items of work. The engineer should use the criteria described in Subsection C-104 of the FHWA Michigan Operations Manual, to determine whether the request is valid to warrant further consultation with the FHWA Area Engineer.  The engineer should be aware that waivers are sometimes granted; however, it is on a project case-by-case basis.  The consideration of a cost savings approach for the differential between domestic and foreign products is not typically supported by FHWA as grounds for a waiver.
 
 
 
 
 
 
 
Approval Authority
 
 
 
The engineer will coordinate the waiver review with their chain of custody, including their FHWA area engineer, on whether an item should be subsequently processed through the FHWA Michigan Division.  It is important to note: The engineer does not have the approval authority to waive any of the Buy America requirements delegated to the department for any federal-aid funded contract.  All approvals must be obtained from the FHWA Michigan Division Office.
 
 
 
 
 
 
 
Compliance
 
 
 
The practice of making otherwise federal-aid eligible items non-participating (state or local agency funded) to circumvent the Buy America requirements is unacceptable and should not be approved in federal-aid projects.  The engineer must make every effort to be in full compliance.  The engineer must secure the necessary manufacture certification paperwork from the contractor to meet the contract requirements for the Buy America provisions prior to making payment for the items of work

The Federal Highway Administration (FHWA) requires contractors to prominently display posters in areas available to employees and applicants for employment during the life of the project.  Complete form 1967, Jobsite Poster Inspection Checklist, during the start of the construction project.  This form is an aid to make sure that the contractor is following the proper procedure, as required by specification.  The completed form must be in the project files.

Staging and Storage Areas
 
 
 
Contractors are not permitted under any circumstances to use any public recreation area as a staging area, marshalling yard, storage facility, or any other construction support unless approved by the Michigan Department of Transportation (MDOT) and the Federal Highway Administration (FHWA) prior to project letting.  Public recreation areas include: parks, trails, recreation areas, game areas, wildlife and waterfowl refuges, playgrounds, golf courses, ball fields, etc.; which are publicly owned (local, state or federal).  These areas are protected by Section 4(f) of the United States Department of Transportation Act of 1966. Federal law requires there be no use of these areas unless: 1) There is no prudent and feasible alternative; and, 2) All possible measures are taken to minimize harm to the area if there is a use. Compliance with federal law requires these criteria be met and their documentation be approved by the FHWA prior to the project letting.
 
 
 
 
 
 
 
Any and all negotiated agreements between the contractor and the owner of the public recreation area are not to be considered valid by MDOT. MDOT’s Environmental Section manager is to be immediately notified if project staff are informed of any negotiations or receive a copy of an agreement. Furthermore, the contractor is to be immediately notified that these discussions and/or agreements are not valid, will not be honored by MDOT, and non-compliance can result in penalties up to and including termination of the construction contract as well as immediate restoration of the public recreation area at the contractor’s cost.

Filter bag requirements are specified in subsections 208.03.D.3 and 208.03.C.3 in the 2003 and 2012 Standard Specifications for Construction, respectively. Filter bags should be placed in a well-vegetated, relatively flat upland area, away from any watercourse. For those projects with limited right-of-way where the discharged water may not be adequately clarified before it enters the waterway, one or more gravel filter berms should be installed between the filter bag and the watercourse to provide additional filtration. The contractor must furnish and install filter bags per the specifications and maintain proper filtration prior to discharge to any watercourse.

• Filter bags with holes or tears are not acceptable
 
 
 
• Place filter bags in an upland area
 
 
 
• Pumps should be sized based on filter bag capacity

Mandrel Testing of Corrugated Polyethylene Pipe (CPE)
 
 
 
During the preconstruction meeting it should be pointed out if CPE pipe is installed, at least 50 percent of each size must be mandrel tested.  The engineer should select those runs of pipe that may have been problematic during installation or in areas subjected to a high volume of construction traffic.  The mandrel testing should be conducted 5 to 10 work days prior to paving.  Prior to testing, the contractor must provide an approved mandrel which is appropriate for testing CPE pipe (mandrel size for PVC is smaller). The mandrel must have at least nine points and be 95 percent of the nominal diameter of the pipe. Per AASHTO M 294 Corrugated Polyethylene Pipe, nominal pipe diameter is defined as 12”, 15”, 18”, etc. Mandrels can be a fixed size or a variable size and are commercially available for most pipe sizes up to 48”. The diameter of the mandrel, whether it is fixed or variable size, must be verified with a proving ring. As a rule of thumb, the length of the mandrel should be equal to or greater than the diameter.
 
 
 
 
 
 
 
The pipe should be cleaned thoroughly before testing. Even the smallest amount of debris can hinder the passage of the mandrel. During the test, the mandrel must be pulled through the pipe by hand. The use of a winch, come-along or other mechanical means is prohibited. If the mandrel can not be pulled through the pipe, the test fails and corrective action is required.  Prior to any corrective action, the contractor may opt to video the pipe to determine the cause of the mandrel not passing through or attempt to clean the pipe again and re-run the mandrel test. If the test still fails, the CPE pipe may be removed and reinstalled providing it has been inspected for damage and approved for re-use. The other option is to remove and replace the damaged pipe with new CPE or other acceptable pipe type. If CPE pipe is used it must be mandrel tested after installation. The use of a pipe “re-rounder” is not an acceptable method for corrective action. Video taping of CPE pipe for acceptance should be conducted only after passing mandrel tests have been achieved.
 
 
 
 
 
 
 
Two forms have been created to document CPE pipe installation and mandrel testing results for sewers and culverts, form numbers 1974 and 1975, respectively. All CPE pipe installed on a project must be recorded on these forms along with mandrel test results. This information must also be documented on the IDR. Upon completion of the form(s), the original version must be placed in the project files (file folder # 304 Drainage Items Testing) and a copy sent to the Grading and Drainage Engineer at C&T for inclusion in the mandated annual report to the legislature.

HMA Material Transfer System
 
 
 
The following information discusses an acceptable paver hopper insert, as required by any special provision, or other material transfer device specifications requiring a paver hopper insert.  A paver hopper insert with a minimum capacity of 15 tons must be provided and installed in the paver for the system and must be kept at least 1/3 full at all times.  Current specification does not allow the paver hopper to be modified to meet the tonnage capacity required of the hopper insert.  Hopper inserts are designed in such a manner that they provide mass flow of the HMA to the flow-gates of the paver.  Mass flow of HMA should eliminate any dead zones where material accumulates and cools, and eventually may slough off into the flow-gates of the paver creating chunks in the paving operation.  Simple modifications to a paver hopper insert to increase the capacity do not allow for mass flow; however, simple modifications can create dead zones and allow sloughing of cold material into the flow-gates.
 
 
 
 
 
 
 
The paver hopper insert must be constructed of a material that is stiff enough to maintain its shape when self supported.  Section 502.03 of the 2003 Standard Specifications for Construction state in part the following, “Provide safe and environmentally acceptable equipment that can produce a specification pavement”.  The contractor will supply the engineer with the details of the hopper, including material type, dimensions and capacity prior to being used on the project in accordance with Section 104.01 of the 2003 Standard Specifications for Construction.  The engineer will approve the use of any proposed paver hopper insert prior to use on the project.  These requirements must be enforced to maintain statewide consistency of specifications.
 
 
 
 
 
 
 
Hot Mix Asphalt (HMA) Paving – Seasonal Limitations
 
 
 
Adherence to seasonal limitations must be followed according to the applicable standard specifications and contract documents, unless a contract authorization is issued to allow the contractor to pave.  A contract modification is required when there is a change in contract conditions.  The modification must document and state under what conditions the contractor may pave, and the contractor must meet these conditions each day of paving.  All other specification requirements, including weather, temperature and compaction, remain in place.  These requirements can be found in Section 502.03.J and Table 502-1 of the standard specifications.

Shipping Tie Wires on Transverse Joint Assemblies
 
 
 
Field investigations conducted by the department in the 1980’s concluded that there were cases where early-age transverse cracking of the pavement had occurred as a result of excessive stresses being built up at the joint assembly when the shipping tie wires were not removed prior to paving. However, a recently conducted national study documented that the additional stresses resulting from not removing the shipping tie wires would be of insufficient magnitude to impact early-age transverse cracking of the pavement. The study further concluded that by cutting the shipping tie wires, there is much more likelihood that the transverse joint assembly will be distorted under the loading imposed by the plastic concrete during paving, which could result in misalignment of the dowel bars and, ultimately, potential lockup of the in-service pavement joints.
 
 
 
 
 
 
 
Contractors are not to cut and remove shipping tie wires after the assemblies are staked onto the grade.  Further, as noted in Standard Plans Series R-40 and this FUSP, the entire joint assembly is to be rejected if the nominal diameter of the shipping tie wires exceed 0.177 inch (approx. 3/16 inch). Since cutting shipping tie wires may result in distortion of the assembly under loading (as described above), transverse joint assemblies that arrive at the project site with oversize shipping tie wires should be rejected. It is often necessary to dewater from within a cofferdam to obtain a dry work area. Subsection 704.03.C of the 2003 and 2012 Standard Specifications for Construction describe the requirements for using a geotextile filter bag (filter bag) during dewatering operations. When the work requires pumping of water that may contain silt and sediment, it must be properly treated as described below and per specification. The engineer should inspect the water prior to allowing discharge directly into a watercourse and monitor during the course of pumping to ensure it remains silt and sediment free. Welding issues noted in the field have included welders and contractors without welder certifications available at the job site, or welders working out of position from their certification, which conflicts with 707.03.D.8.d. of the standard specifications
 
 
 
There were also issues with welds not meeting the welding requirements per 707.03.D.8.b. This includes welders not removing the slag from their welds, improper grinding, piles that had a poor fit up, and misalignment of the pile sections. There were a considerable number of welds that did not meet American Welding Society (AWS) Code for visual inspection such as arc strikes, cold lap, undercut, and underfill.
 
 
 
 
 
 
 
Some welders were not aware of the weld requirements such as weld lengths, locations, weld size, weld type, and root preparation including back gouging of the welds.
 
 
 
 
 
 
 
There were also issues with improper storage of low hydrogen (E-7018) welding rods according to 707.03.D.8.b of the standard specifications, such as leaving weld rods out for more than two hours, open partially used cans of welding rod on site, hot boxes with the tops left open, welding in the rain, welding on wet steel, welding rods sitting on the ground, hot boxes not plugged in overnight, hot boxes not plugged in during work or generators not running.
 
 
 
 
 
 
 
Include documentation of poor welds and the welder who performed the welds on Form 1122B, Inspector’s Daily Report (IDR). Attach photos as necessary. Most welds can be accepted or rejected based on visual inspection conducted during and after welding. If there is question as to the acceptability of a finished weld, non-destructive testing can be performed to verify.

Material and Application Verification Guidelines explain documentation requirements, how to use Form 1585, contractor yard visits, and plate sampling.  By following this guideline, pavement marking verification practices will be performed consistently statewide.
 
 
 
 
 
 
 
Pavement Marking Materials Reporting Requirements
 
 
 
Subsection 105.01 of the 2003 Standard Specifications for Construction requires the contractor to provide a material source list for all materials incorporated into the project.  This information should be provided at the preconstruction meeting.  The contractor must include the product name as shown on the Qualified Products List (QPL) as well as the product manufacturer.
 
 
 
 
 
 
 
Section F of the Materials Quality Assurance Procedures Manual requires the placement of a label or copy of the label, tag, etc. in the project file.  A notation on an Inspector’s Daily Report (IDR) may be completed in lieu of a label.  Include the product and manufacturer on the IDR, and note that the labeling of the material was checked and verified.

Pole band clamps must be stamped with identification that is traceable back to the manufacturer, which can be used to determine the month and year of clamp manufacture. The stamping method must be a low stress stamping method. The minimum inside bend radius for cold formed members must be at least 1.5 times the material thickness (or 3/8 inches).
 
 
 
 
 
 
 
The contractor must submit a Certificate of Compliance to the engineer stating that the pole band clamp meets the requirements in Special Detail SIG-010-A. The contractor must provide one clamp assembly per lot per size to the engineer for testing as a basis of acceptance prior to use on the project. Materials Testing Documentation Requirements for Pre-Qualified Aggregates
 
 
 
All aggregates supplied to MDOT projects from pre-qualified sources must have quality assurance (QA) testing conducted by MDOT personnel at frequencies established in the MDOT Materials Quality Assurance Procedures Manual, Section C6, part 7, and in the Materials Acceptance Requirements Table in the Materials Source Guide. Region materials personnel will monitor and conduct quality assurance testing for pre-qualified aggregate sources within their jurisdiction.
 
 
 
 
 
 
 
As part of the materials testing documentation, the delivery/project engineer must verify and document that the QA testing has been completed for the material received from pre-qualified aggregate sources. The delivery/project engineer will need to contact the region’s material supervisor for verification. The materials supervisor will confirm that QA testing has been properly completed and provide the delivery/project engineer with written documentation for the project files to be placed in file folder 306. Additional documentation required for pre-qualified aggregates include properly completed delivery tickets. The tickets must include the following:
 
 
 
• MDOT aggregate source number;
 
 
 
• Date of shipment;
 
 
 
• Control section and job number;
 
 
 
• Concrete plant number (if applicable);
 
 
 
• Michigan series number and class letter of aggregate;
 
 
 
• Weight or volume shipped;
 
 
 
• Suppliers name, telephone number and location.
 
 
 
In addition, the following statement shall be printed or stamped on each ticket: “I attest that aggregate as delivered from this pre-qualified source meets specification requirements for the listed Michigan series and class for quantity stated. Bridge Mounted Light Standard Anchor Bolt Assemblies
 
 
 
All light standard anchor bolt assemblies, whether supplied and installed by the contractor or by another agency, must meet MDOT specifications when installed on department bridges.  Although bridge mounted light standards may be inspected during scheduled bridge inspections, they do not receive scheduled inspections throughout their service life similar to tower lights or cantilever sign supports.  The foundation anchor bolts are critical to ensuring the structural capacity of the light standard throughout the design life.  The following specifications must be adhered to:
 
 
 
 
 
 
 
MDOT Standard Plan B-103-E contains specifications for anchor bolt diameter, length, embedment and couplers.  Anchor bolts and associated hardware must conform to section 908.15 of the standard specifications, and must be sampled in accordance with section 908.15 of the Materials Acceptance Requirements Table in the Materials Source Guide, at a frequency of one per heat per diameter per project.  Any anchor bolt assemblies containing deviations from the standard plans should be called to the attention of the Bridge Field Services Section in Lansing.  Samples from non-standard anchor bolt assemblies should be accompanied by shop drawings and all samples should be accompanied by test data certifications.
 
 
 
 
 
 
 
Anchor bolt installation and tightening using the “turn of nut” procedure must be conducted in accordance with Section 810.03.N of the standard specifications.

Current News

New FieldManager Helpdesk Number

The FieldManager Helpdesk activated its new phone number, 1-844-4MI-MDOT (1-844-464-6368). On the new phone menu, you can reach the FieldManager Helpdesk by selecting option 2 for AASHTOWare and then option 3 for FieldManager. The old number was deactivated on May 1st. The new number is part of the new unified help desk solution, MI MDOT Software Support, used to track and resolve software related issues from customers.

Preamble

This manual provides guidance to administrative, engineering, and technical staff. Engineering practice requires that professionals use a combination of technical skills and judgment in decision making. Engineering judgment is necessary to allow decisions to account for unique site-specific conditions and considerations to provide high quality products, within budget, and to protect the public health, safety, and welfare. This manual provides the general operational guidelines; however, it is understood that adaptation, adjustments, and deviations are sometimes necessary. Innovation is a key foundational element to advance the state of engineering practice and develop more effective and efficient engineering solutions and materials. As such, it is essential that our engineering manuals provide a vehicle to promote, pilot, or implement technologies or practices that provide efficiencies and quality products, while maintaining the safety, health, and welfare of the public. It is expected when making significant or impactful deviations from the technical information from these guidance materials, that reasonable consultations with experts, technical committees, and/or policy setting bodies occur prior to actions within the timeframes allowed. It is also expected that these consultations will eliminate any potential conflicts of interest, perceived or otherwise. MDOT Leadership is committed to a culture of innovation to optimize engineering solutions. The National Society of Professional Engineers Code of Ethics for Engineering is founded on six fundamental canons. Those canons are provided below. Engineers, in the fulfillment of their professional duties, shall:

  1. Hold paramount the safety, health, and welfare of the public.
  2. Perform Services only in areas of their competence.
  3. Issue public statement only in an objective and truthful manner.
  4. Act for each employer or client as faithful agents or trustees.
  5. Avoid deceptive acts.
  6. Conduct themselves honorably, reasonably, ethically and lawfully so as to enhance the honor, reputation, and usefulness of the profession.


This manual has been revised throughout to incorporate changes brought about by the release of the 2012 Standard Specifications for Construction and by progress in equipment, construction practices, and materials. The format has been established to follow the standard specification outline with divisions and sections set up to facilitate revision and addition of new information as needed.
